What to expect by age
0-4 months
Full-term babies are born with iron stores that typically last about four to six months. Breast milk contains a small amount of highly bioavailable iron, while infant formula is fortified with iron. No additional iron-rich foods or supplements are needed at this age for most babies. Premature babies may need an iron supplement starting at two weeks of age, as they are born with lower iron stores.
4-6 months
As your baby approaches six months, their iron stores begin to run low. This is one reason why around six months is the recommended time to introduce complementary foods. The AAP recommends that iron-rich foods be among the first foods introduced. Iron-fortified single-grain infant cereal (such as oat or rice cereal) mixed with breast milk or formula is an easy first iron source. Pureed meats are also an excellent early food, as they contain heme iron which is the most easily absorbed form.
6-9 months
Aim to include an iron-rich food at most meals. Good options include iron-fortified infant cereal, pureed or finely minced chicken, turkey, beef, pork, or fish, mashed beans and lentils, tofu, and egg yolks. Pair these with vitamin C-rich foods like sweet potato, broccoli, strawberries, or citrus to boost iron absorption. If your baby is breastfed and not eating much solid food yet, talk to your pediatrician about whether an iron supplement is appropriate.
9-12 months
By this age, your baby should be eating a variety of iron-rich foods regularly. Finger foods like small pieces of soft-cooked meat, bean patties, scrambled eggs, and strips of tofu are great options. Iron-fortified cereals can be mixed into other foods like yogurt or mashed banana. The AAP recommends screening for iron-deficiency anemia around 12 months of age.
1-3 years
Toddlers need about 7 mg of iron per day. Good sources include red meat, poultry, fish, beans, fortified cereals, spinach, and dried fruits like raisins. Be aware that drinking too much cow's milk (more than 24 ounces per day) can interfere with iron absorption and is one of the most common causes of iron-deficiency anemia in toddlers. If your toddler is a picky eater, a multivitamin with iron may be helpful.

- Your baby's stool becomes darker or greenish after starting iron-rich foods or supplements, which is a normal color change

- Your baby is breastfed, is eating very little solid food by 9 months, and you are concerned about iron intake
- Your baby was born premature or had a low birth weight, which increases the risk of iron deficiency

- Your baby appears very pale, is unusually tired or irritable, has rapid breathing, or craves non-food items like dirt or ice, which are signs of significant iron-deficiency anemia

When to Introduce Allergens to Baby
Current guidelines recommend introducing common allergens (peanut, egg, cow's milk products, tree nuts, wheat, soy, fish, shellfish, sesame) starting around 4-6 months when your baby is developmentally ready for solids. The landmark LEAP study showed that early introduction of peanuts (by 4-6 months) reduced peanut allergy risk by 80% in high-risk infants. Do not delay allergens - the old advice to wait until 1-3 years has been reversed because early exposure actually prevents allergies.
I'm Worried My Baby Is Aspirating During Feeds
Aspiration means liquid or food enters the airway instead of the stomach. Occasional coughing during feeds is common and does not usually indicate aspiration. True aspiration is less common and may present as recurrent respiratory infections, a wet or gurgly voice after feeds, or chronic cough. If you are concerned, a swallow study can provide a definitive answer.
Could My Baby Be Aspirating During Feeding?
Aspiration occurs when food or liquid enters the airway instead of the esophagus. Signs include coughing or choking during every feed, a wet or gurgly voice after eating, recurrent chest infections, and breathing changes during meals. Silent aspiration can occur without obvious coughing. If you suspect aspiration, contact your pediatrician as a swallowing study can diagnose it.
Baby Biting Nipple While Nursing
Biting during breastfeeding is a common challenge, especially when babies start teething. It can be startling and painful, but it is almost always a phase that can be managed. Babies cannot actively nurse and bite at the same time because their tongue covers the lower teeth during proper sucking. Biting typically happens at the beginning or end of a feed when the latch is not active. With some gentle strategies, most babies learn quickly that biting ends the feeding session.
My Baby Keeps Clamping Down on the Spoon
Clamping down on the spoon is very common, especially during teething or when babies are learning new oral motor skills. It is often a sensory exploration behavior rather than a feeding problem. Using a soft silicone spoon and placing food on the front of the spoon can help.
How Can My Baby Get Enough Calcium Without Dairy?
If your baby cannot have dairy due to allergy or intolerance, there are many other calcium sources. These include calcium-fortified foods, broccoli, kale, tofu made with calcium sulfate, beans, calcium-fortified plant milks (after 12 months), and sardines. Breast milk and formula provide adequate calcium before 12 months. If dairy-free after 12 months, planning is important.
